Hypogonadism, the clinical term for low testosterone, is a progressive condition that doesn't resolve on its own. It falls into two categories depending on where the problem originates, and that origin changes which treatments are appropriate and what long-term management looks like. Both are well understood and treatable, but they aren't interchangeable.
Primary Hypogonadism
Your testes aren't producing enough testosterone on their own. Whether this is due to injury, infection, or something you were born with, treatment means replacing what your body can no longer make, which is where TRT comes in.
Secondary Hypogonadism
Your testes may still be capable of producing testosterone, but your brain isn't sending them the signal to do it. This can develop from aging, obesity, chronic stress, or certain medications. Treatments work by restoring that signal.

Most clinics prescribe testosterone. Few bother figuring out if you need it.
Pill mills run a questionnaire, check a single lab value (if that), and write a prescription. That can mask an underlying condition, miss contraindications, or put you on a one size fits all protocol that was never built around you.
Inaccurate Testing
Most clinics use what's called an immunoassay to measure testosterone. It's cheaper, but it's less reliable, especially at the ranges where accuracy matters most. The gold standard is LC-MS/MS, the same methodology used in clinical research for its precision, but most clinics don't use it because it costs more.
Incomplete Labs
Total testosterone is one number, and on its own it tells you almost nothing. You need to see the full picture of hormones, metabolic health, and organ function, to understand what's driving the problem. Most clinics check one or two values and stop there.
Overprescribing
200mg of testosterone cypionate every two weeks and anastrozole. That's what most men receive regardless of their lab results, symptoms, or their goals. Sometimes the answer isn't a prescription at all. It could be a vitamin D deficiency, weight loss, better sleep, or managing stress. The industry knows this, it just isn't profitable to tell you.
